Service Agreement – Crime & Trauma Scene Remediation
At Biohazard Busters, LLC, we understand the sensitive nature of biohazard cleanup and provide discreet, professional remediation services. Our agreement ensures transparency regarding our services, responsibilities, and payment terms.
1. Financially Responsible Party (FRP) Information
The Financially Responsible Party (FRP) is the individual or entity authorizing remediation services. The FRP must provide:
- Full Name & Affiliation (e.g., Property Owner, Executor, Business Representative)
- Contact Information (Phone, Email, Address)
- Property Details (Suite/Apt, City, State, Zip Code)
2. Job Site Information
The Job Site is the location where remediation will be conducted. The FRP must provide:
- Site Address (Including Suite/Apt)
- Point of Contact Information (Name, Phone, Email)
3. Permission to Enter Job Site
By signing this agreement, the FRP certifies that they have the legal authority to permit Biohazard Busters, LLC to enter the property for assessment and remediation. This includes:
- Inspection for estimating costs
- Removal of biohazardous materials (blood, bodily fluids, and other contaminants)
- Complete disinfection and decontamination of the affected area
4. Disinfection Services Estimate
Our trained biohazard removal technicians will:
- Identify, remove, and dispose of biohazards in compliance with regulations
- Use environmentally safe equipment and chemicals
- Provide an initial cost estimate, which may vary based on additional contamination found during cleanup
🔹 Additional Costs: If unexpected hazards (e.g., hidden contamination, mold, structural damage) are found, we will discuss any extra charges before proceeding.
5. Disposal of Contaminated Items
The FRP may authorize Biohazard Busters, LLC to:
✅ Disinfect, remove, and dispose of contaminated furniture, appliances, and fixtures
✅ Remove contaminated carpets, padding, and subflooring
✅ Disinfect and seal porous surfaces (e.g., concrete, drywall)
6. Checklist of Affected Areas
Our remediation services cover:
🏠 Living Areas – Living Room, Dining Room, Hallways, Offices
🛏 Bedrooms – Master Bedroom, Additional Bedrooms
🛁 Bathrooms – Full and Half Baths
🍽 Kitchen & Pantry
🏢 Additional Spaces – Basement, Attic, Garage, Outbuildings
7. Estimated Costs & Payment Terms
💰 Service Costs: Based on technician hours, contamination level, and required materials.
💳 Payment Requirements:
- 50% deposit required before services begin
- Remaining balance due upon completion
- Payments accepted via cash, check, or credit card
8. Description of Services
Our certified biohazard remediation technicians will thoroughly clean, sanitize, and dispose of hazardous materials in compliance with state and federal regulations.
9. Terms & Conditions
- Authority to Enter Premises – FRP grants Biohazard Busters, LLC access for remediation.
- Personal Property Disposal – Irreparably contaminated items will be disposed of unless otherwise noted.
- Insurance Assignment – FRP may assign insurance benefits to cover services.
- Liability Release – Biohazard Busters, LLC is not responsible for damages beyond biohazard remediation.
- Legal Fees – The FRP agrees to cover any legal fees related to payment disputes.
10. Special Considerations
- Residual & Psychological Odors – Additional deodorization may be required.
- Hazardous Materials – Our services cover only biohazards, not asbestos, chemicals, or illegal substances.
- Broken Glass Cleanup – While we make every effort to remove all shards, we cannot guarantee 100% removal.
